Hi, I’m Dr. Rafael Abril, board certified bariatric surgeon in Mexicali, Mexico, and today we are going to talk about the gastric sleeve vs gastric bypass and when one of those is necessary. First of all, both of them are great options for obesity treatment, both control control obesity long term. These two surgeries will have great effect in that problem, but the question is, which one is better? Well, first of all, we ourselves are very different, we have different needs. This is one thing we need to know.
But we’re going to start talking about the gastric sleeve surgery in Mexicali. It is a reduction of your stomach size, resulting in a stomach 20% the size of the original one. This makes your stomach look like a long tube. The 80% of the stomach that was removed has a hormone called Ghrelin, which is the one that creates craving and appetite. Thus, with the gastric sleeve, craving for food is pretty much controlled. And, of course, there’s the restriction of food itself. This is the way having gastric sleeve surgery in Mexicali controls weight loss.
The gastric bypass in Mexicali is done very different. First you separate the stomach making a new small pouch on the upper part of the stomach, and then you connect the intestine on the upper part of this pouch. When you eat there is a food restriction because there is a small pouch and you jump approximately 100 to 150 cm of intestine of digestive tract where you do not absorb calories and sometimes less iron. That means, of course, that you must take some supplements with the gastric bypass.
Both the gastric sleeve in Mexicali and the gastric bypass in Mexicali have a similar rate of weight loss, but the bypass has a little bit of advantage with people that suffer from diabetes or metabolic problems associated with obesity. In the gastric sleeve you are going to have a remission of the diabetes, the thing is you don’t need to have medication or insulin in about 70% of the cases. With the gastric bypass is going to be like 90% of the times with people with diabetes. So, if you suffer from diabetes, you need to consider that the gastric bypass surgery in Mexicali is going to be more effective for treating both weight loss and diabetes, although as you read already, gastric sleeve is not bad at all.
When you have a difficult reflux disease, bypass is going to cure it. In fact, this procedure started as a surgery for reflux control. By separating the stomach from the upper part there is not going to be any reflux to the esophagus. So this is something else you need to consider when you have obesity.
Those with a sweet tooth will see greater changes with the bypass as well. But remember, if you’re having weight loss surgery to begin with, you need to take a different approach in the way you eat. Is a whole new life. You do not need to continue eating the same things you ate before surgery.
At the end, both procedures have pretty much the same percentage when it comes to weight loss. The pluses on the bypass surgery in Mexicali come for people with diabetes and those with reflux issues. The pluses with the gastric sleeve is that people with obesity will not see their anatomy dramatically altered, they have the same digestive tract even though the stomach is smaller. Supplements are rarely needed in the gastric sleeve surgery in Mexicali. And then, if you think about this, and it may not be a priority for you, but the gastric sleeve may convert into a gastric bypass if needed.
Both surgeries are very good, it is a choice you need to make together with your surgeon and his surgery team.
